Job Title: PharmacistSpecialty: Pharmacy Services Clinical, Inpatient, and OutpatientLocation:
  • Phoenix VA Health Care System (PVAHCS)
  • 650 E. Indian School Rd., Outpatient Pharmacy, Building 8, Room A1215
  • Inpatient Pharmacy, Building 1, Room 3108
  • 32nd Street Outpatient Pharmacy, Phoenix, AZ 85012
  • Note: The first two sites are on the main campus; the third site is one lane away from the main campus.
Duration: 1 year contract with the possibility of a 4 year extensionShift Schedule: Facility operates 24/7.
  • Pharmacists may be scheduled for:
  • 8, 11, or 12-hour shifts
  • Day: 06 AM 02:30 PM
  • Evening: 2 PM 10:30 PM
  • Overnight: 7 PM 06 AM or 8 PM 6 AM
  • Weekend and holiday shifts may be required based on staffing needs.
License/Certifications:
  • Unrestricted license to practice pharmacy in any U.S. State, Territory, or the District of Columbia.
  • Must be licensed to handle controlled substances.
  • No history of license restrictions, suspensions, or revocations.
  • Must pass credentialing and privileging requirements via VetPro (60-day process).
Education:
  • Graduate of an ACPE-accredited College or School of Pharmacy.
  • Minimum degree: B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.)
Minimum Qualification/Experience:
  • 24 months of relevant experience within the past 36 months preferred.
Responsibilities:
  • Deliver full-spectrum pharmacy services per VA standards.
  • Dispense prescribed medications and advise patients on proper usage.
  • Evaluate medication orders for:
  • Drug interactions
  • Dosing appropriateness
  • Allergies
  • Duplications and contraindications
  • Counsel patients regarding adverse effects and drug interactions.
  • Recommend therapeutic alternatives aligned with VA formulary.
  • Ensure compliance with state and federal drug control regulations.
  • Maintain medication security and inventory control.
  • Input and verify prescriptions in the VA's electronic medical record (VistA/CPRS).
  • Report outcomes and participate in continuous quality improvement initiatives.
  • Collaborate with healthcare providers to enhance patient care and medication safety.
  • Maintain professional and cooperative interactions with patients, providers, and staff.
