- Dispense medications prescribed by physicians and other health practitioners in compliance with VA policy and regulations.
- Advise medical staff on medication selection, dosages, potential interactions, and side effects.
- Counsel patients on medication use, adverse reactions, and drug interactions, and provide education on durable medical equipment and home health supplies.
- Evaluate provider medication orders for appropriateness, including reviewing for duplications, incompatibilities, allergies, and possible therapeutic problems (e.g., dosing, strength, intervals).
- Communicate and recommend alternatives to questionable medication therapies according to VA formulary and CPRS guidelines.
- Ensure all medication orders are accurately entered and verified in the electronic patient medical record (VistA).
- Maintain compliance and strict security for all controlled substances, uphold perpetual inventory control, and accurately document drug handling in accordance with VHA Handbook.
- Demonstrate positive interpersonal skills and foster a supportive work environment with providers, patients, and staff.
- Graduate of an ACPE-accredited College or School of Pharmacy (B.S. Pharmacy and/or Pharm.D.).
- Must hold a full, current, and unrestricted pharmacist license in any U.S. state, territory, or the District of Columbia.
- Licensed for controlled substances dispensing.
- Proficient in English (reading, writing, speaking).
- Ability to verify competency, skills, certification, and education as required.
- No history of restricted, suspended, or revoked licensure.
