- Will work under the direction of the Home Health Registered Nurse and Clinical Manager.
- Will work with all age groups providing independent hands-on patient care in Trumbull
- Providing personal care, such as bathing, dressing and toileting assistance, and ensuring patient comfort and safety.
- Educating patients and their families about their conditions, treatment plans, and self-care techniques.
- Charts and documents all patient care according to state regulations. Will provide after hours on-call coverage on a rotating basis.
- Home Health experience is preferred.
- Must Possess excellent assessment and communication skills along with up-to-date computer skills.
- Must be a Licensed Practical Nurse in the State of Ohio without restrictions.
- Must have a valid driver's license with an acceptable driving record and proof of insurance.
- Must be willing to travel and have reliable transportation.
