Summary: Home Health LPN Provides nursing care to patients in their homes, helping to manage chronic conditions, recover from illness, or maintain independence.Essential Duties and Responsibilities include the following. Other duties
may be assigned.
  • Will work under the direction of the Home Health Registered Nurse and Clinical Manager.
  • Will work with all age groups providing independent hands-on patient care in Trumbull
County, Mahoning County, and Northern Columbiana County. * Will provide wound care, ostomy care, urinary and suprapubic catheter care and changes, pleurx catheter care, manage complicated dressings, medication education, venipuncture,and perform other nursing procedures as required within the scope of practice. * Provides independent hands-on patient care to include vital signs, medication management, and monitoring for changes in condition.
  • Providing personal care, such as bathing, dressing and toileting assistance, and ensuring patient comfort and safety.
  • Educating patients and their families about their conditions, treatment plans, and self-care techniques.
  • Charts and documents all patient care according to state regulations. Will provide after hours on-call coverage on a rotating basis.

  • Home Health experience is preferred.
  • Must Possess excellent assessment and communication skills along with up-to-date computer skills.
Certificates, Licenses, Registrations
  • Must be a Licensed Practical Nurse in the State of Ohio without restrictions.
  • Must have a valid driver's license with an acceptable driving record and proof of insurance.
  • Must be willing to travel and have reliable transportation.
