Description :Job Summary:The School Based Health Coordinator (“SBHC”) will serve as the “bridge” between the K-12 and Healthcare sector partners working together to improve the academic success and health of students and families. Each SBHC will become a trusted resource and point of contact for students and parents/guardians at each school site. Care should be taken to not undermine the trust component necessary for students and parents' sense in comfort accessing help.The SBHC will carry a prioritized caseload of students/families and will facilitate access to appropriate, direct and/or telehealth services to address the combined health, social/emotional needs of students to improve student health and academic engagement. Beyond this daily prioritization of health services, the SBHC will collaborate with other school staff and community partners to lead or support value added student development activities and events.Job Specific Competencies
  • Facilitate access to health and wellness services available from CVCH and other community partners leveraging virtual modalities and in person care where
  • Lead the identification and assessment of student health needs and stratification of students by Tier 1, 2 & 3 for priority of services in collaboration with the District, and CVCH staff/providers.
  • Coordinate with community partners to provide parent workshops and lead/attend family events, including back-to-school events, family nights that pertain to health topics (i.e. mental health, childhood obesity, worker safety, community resources, hygiene, dental, vaccines, ).
  • Collaborate with the District administrators, teachers, and staff, including 504 planners, school nurses, health services staff, and counselors, to identify low income and migrant students in need of service, including migrant students with high absenteeism and chronic
  • Collaborate with District counselors and other professionals to identify student and family health needs (physical, social-emotional-behavioral, and others).
  • Prioritize students and develop a caseload of 40-50 students in coordination with CVCH providers/staff, migrant student advocates, biopsychosocial supports and other student support personnel and connect at least two (2) times per quarter with 80% of priority for Tier 1 identified students
  • In collaboration with CVCH staff/providers and District staff, communicate and coordinate with students, families and district staff regarding student needs and migrant health services where applicable.
  • Coordinate with community resources as appropriate for student and family care referrals, including filling the requirements for the migrant physical exams, immunizations, sports physicals, etc. to support engagement in academic activities.
  • Document SBHC data related to student assessment and interventions in case log. Leverage school and health record data to track Key Performance Indicators for program evaluation.
  • Meet with District administrators, school nurse and school counselor on as need basis, but at least quarterly, for communication and collaboration regarding SBHC students and OSPI.
  • Where applicable, participate in training/meetings with the District’s federally funded Special Programs leaders as appropriate for enhancing SBHC skills and program knowledge.
  • Participate in training, reporting and data collection efforts as required by OSPI.
  • Utilize the agreed universal health screening tools, for the purpose of identifying unmet or ongoing health needs of all students and families.
  • Help coordinate other student and parent development activities in collaboration with the District, CVCH and other community partners.

Job Specifications * Education: Associates Degree or equivalent combination of education and work experience. Bachelor’s degree in Sociology, Psychology, Social Work, Public Health, or a related field preferred.
  • Certification/Licensure: State Certified Translator Preferred.
  • Experience: Previous experience providing administrative and/or intake assistance in an integrated healthcare or social services setting preferred. Demonstrated success in working with children and families.
  • Language Skills: English required.
  • Essential Technical/Motor Skills: Knowledge of computer applications and equipment related to work. Must have basic computer and keyboarding skills and have the ability to enter data within company’s computer system to include strong knowledge in MS Word/Excel; must demonstrate manual dexterity. Exhibit strong customer service skills, strong process improvement background.
  • Interpersonal Skills: Strong interpersonal and communication skills and the ability to work effectively with other staff and management. Demonstrated skill in developing and maintaining productive work teams. Ability to demonstrate personal integrity in all interactions.
  • Essential Physical Requirements: This job is performed mostly in a typical inside, office environment. Essential physical requirements of this job include: light physical effort; repetitive motions of wrists, hands, and/or fingers; standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, and pulling; frequent sitting; lifting and/or moving items up to 50 pounds, with assistance as needed; ability to read forms and computer screens and to read correspondence and other documents.
  • Essential Mental Abilities: Ability to make decisions in line with state and federal regulations; ability to read, comprehend, and analyze documents, regulations, and policies; ability to prepare and submit complete and succinct documents necessary to the job. Ability to assess and evaluate, have attention to detail. Knowledge of auditing and compliance procedures, quality assurance and improvement practices, understanding of the elements of sponsored clinical protocols including consent forms, and reporting requirements. Problem solving and analytical skills are required with a heavy emphasis on detailed analysis of information to support actions.
  • Essential Sensory Requirements: Essential sensory requirements include the ability to: read computer keyboard, monitor, and documents; prepare and analyze documents; read extensively; see, recognize, receive and convey detailed information orally, by telephone and in person; convey accurate and detailed instructions by speaking to others in person and by telephone.
  • Exposure to Hazards: Worker is subject to inside environmental conditions on a frequent basis with moderate noise. Typical working conditions found in most administrative work areas. Worker has contact with consumers and other staff and may be exposed to medical conditions presented by them.
