Franciscan Health Munster Campus701 Superior Ave Munster, Indiana 46321The Patient Navigator Breast Imaging works as a liaison and builds effective working relationships with patients, healthcare providers, and staff to reduce barriers to patient care, meet patient needs, and improve patient outcomes. This position acts as a point of contact for patients and gather pertinent information, listens to patient concerns, and directs patients to the appropriate resources and Healthcare professionals. The Patient Navigator helps plan and coordinate patient care instructions, ensures patient documentation is up to date, and helps patients manage their conditions for better outcomes.WHO WE ARE

  • Identify and track availability of community resource options/bed availability for referral.
  • Obtain disposition recommendations from physicians &/or clinicians.
  • Guide patients while assisting them in navigating their next level of care choices given treatment needs and insurance benefits.
  • Communicate with and coordinate medical necessary information/documentation with receiving in-patient facilities, or out-patient provider reflecting standards of clinical practice within the scope of professional standards.
  • Complete interpretive/discharge summary.
  • Follow up to ensure transition appointments are kept and help to problem solve any barriers to follow through.
  • Attempt re-engagement efforts towards previously identified disposition plans or offer other alternatives, if appropriate.
  • Measure patient satisfaction with disposition process.
  • Assist patient in completing a Safety Plan if suicidal issue is present.
  • Assist patient in completing relapse prevention plan for Substance Abuse &/or Mental Health condition.
  • Identify clinical data needed for dashboard metrics.
  • Review interpretive summaries from each previous admit/episode of care-both inpatient & outpatient.
  • Provide education intervention to patient &/or guardians/supports.
Qualifications
  • Preferred Associate's Degree Radiology
  • Required Certificate Radiology
  • 2 years Imaging Required
  • 2 years Mammography Preferred
  • Mammography (ARRT-M) - The American Registry of Radiologic Technologists (ARRT) required
  • Radiology Technologist License (RADTECH) - State Licensing Board required
  • Basic Life Support Program (BLS) - American Heart Association required
