- Provides education to patients, physicians, nurses, and other healthcare team members as requested.
- MUE, ADR reporting, resolving drug therapy questions, therapeutic drug monitoring, identifying drug interactions and allergies, direct contact with prescribers to clarify major problem orders, and non-formulary drug requests.
- Responds to medical emergencies in department and throughout hospital as needed including Stroke Alerts and Code Blues.
- Assist in procedural sedations as well as rapid sequence intubations to ensure safe and accurate delivery of high risk medications.
- Ensures antibiotic stewardship activities and issues are communicated with the ED practitioners, regarding opportunities to optimize antimicrobial utilization and help reduce inappropriate antibiotic use and antimicrobial resistance.
- Graduate of an accredited College of Pharmacy
- Current Kentucky Pharmacist License
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification or within 6 months of start date
- Completion of PGY-1/PGY-2 ASHP-accredited Pharmacy Residency in area of practice preferred or minimum of 3 years of experience in practice area
- Current board certification or obtained within first year of hire required
