The Scattered Sites Program (SSP) Housing case Manager provides outreach, recruitment and housing stability to qualified residents of St. Martin's Program shelters. This program provides services for people with complex needs and unique strengths who benefit from hands-on case management and resource connections in order to be successful in finding and maintaining housing.All clients participating in St. Martin's shelters suffer from theeffectsof long-term homelessness, and are disabled with mental illness, addiction, physical conditions, and/or advancing age. The Housing case Manager will build relationships and enroll shelter residents in the Scattered Sites Program. This will involve meeting the challenges of resistance to change and limited capacity for negotiating the process of obtaining and keeping housing. Once housed, they will assist clients in successful tenancy. The Housing case Manager will be a bridge between these tenants and building managers, helping to negotiate conflict with the goal of supporting housing for the individual while upholding the lease and mitigating negative impacts. The Housing case Manager will assist tenants in these units in making improvements in health, living skills, income, benefits and quality of life.CCS is committed to making our services, our agencies and our communities free of the divisive and dehumanizing ravages of racism.   • Most residents who will qualify for SSP are "chronically homeless". Outreach will focus on these residents and develop case manager-client relationships to overcome barriers to housing. The Case Manager will work with clients to determine qualification throughdisability.
  • The Case Manager will: Determine eligibility for SSP, interview applicants and assess ability to live autonomously in the community. They will identify special needs in health care, mental health, and alcohol and drug addiction and determine which applicants require further diagnostic screening by mental health or substance abuse professionals. The interview also assesses social skills, life skills, financial needs, job readiness/needs and availability of a positive peer network.
Housing Application and MoveIn:
  • Complete initial assessment: review client rights and initial consent documents, and conduct an assessment identifying the enrollee's strengths, preferences and needs related to housing and needs for support to maintain community integration (including what type of setting works best for the enrollee).
  • Following assessment, with the client, identify and establish short and long-term measurable goal(s), and write out a Housing Service Plan in the clinical record which will address the identified needs as well as the areas which have been a problem for the tenant in maintaining previous housing.
  • Assist with the housing application process: complete paperwork, obtain necessary documentation, and track progress. Serve as the point of communication with apartmentmanagement.
  • Assist clients moving in to their units. Help with completion and understanding of the lease and other paperwork. Help with engaging utilities. Help obtain and transportfurniture and household goods. Assist with engaging any necessary outside services to maintain independent living.
Housing SupportServices:
  • Provide regular case management to tenants after move-in. This includes providing support and guidance during individual meetings with tenants, accompanying tenants to appointments or on errands as necessary, and other activities that help support tenant's transition into housing.
  • Work with new tenants to develop individual growthgoals.
  • Work with clients to assist them in understanding and navigating program rules and responsibilities; assist tenants in understanding the repercussions of not following program structure.
  • Work with building management over lease violations, and help tenants meet terms of notices of lease violations and failure to pay rent. Give input on eviction proceedings.
  • Provide current information about available resources and assist tenants in making application for needed resources including income, health care, employment, documentation, and support services.
  • Provide crisis intervention - which may involve substance abuse, mental health, and medical issues.
  • Arrange access to community treatment resources and support programs by medical, mental health, and alcohol recovery providers.
  • Monitor effectiveness of response and resident satisfaction with resources and servicesreceived.
  • Act as advocate for residents on an on-going basis to ensure consistent service from agencies and systems.
  • Assist in developing a sense of community and support amongst program residents. This includes organizing events, activities, and outings, as well as less formal gatherings.
  • Coordinate services with other staff and outside agencies and help foster an environment of teamwork and professionalism.
Administrative:
  • Actively participate in team and supervisory meetings in order to discuss issues pertaining to clients, program goals, and other program issues.
  • Attend staff meetings for St. Martin's. Attend other meetings and in-service training asrequested.
  • Managetimeandprioritizedutiesin ordertoprovidenecessaryservicestotenants.
  • Keep daily records and maintain case management files in Clarity or other assigned HMISsystem.
  • Turn in paperwork and assignments to the Program Manager on a timelybasis.
oEnsure client selection process, files, services and expenses comply with programcontracts.oFollow program and agency policies and procedures. Provide input to program policies, procedures and goals, as requested.GeneralResponsibilities:
  • Follow guidelines on confidentiality rights of clients and respect theirprivacy.
  • Maintain accurate record of hours worked and turn in timesheets onschedule.
  • Attend trainings required of allemployees.
  • Become familiar with landlord/tenant law and the actions that are permissible forlandlords.
  • Contribute to the mission of Catholic Community Services and Catholic HousingServices.
  • Contribute to and support a positive, team-oriented workenvironment.
MinimumQualifications
  • BA/BS degree, preferably in a related field. (See Substitute Qualificationsbelow.)
  • Two years related experience, which includes providing linkage services, advocacy for low-income populations, and working with special need populations.
  • Knowledge of mental health, substance abuse, and issues resulting from aging, and a demonstrated ability to work with these issues.
  • Washington state issued driver’s license and a clean driving record
  • Demonstrated problem solving skills: assessment, planning, intervention, implementation, and crisis response.
  • Strong oral and written communication and team buildingskills.
  • Ability to work cooperatively with residents, program team and communityprofessionals.
  • Ability to work independently and responsibly, with supervisor located in a differentbuilding.
  • Strong organizational and record keeping skills with ability to maintain concise client records, notes and receipts to assure compliance standards.
  • Good computerskills.
  • Ability to work within the mission, goals, and objectives of catholic CommunityServices.
  • Ability to travel to various sites. Valid Washington State Driver’s Licenserequired

  • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.
  • Criminal history background checks are required prior toemployment.
PreferredQualifications
  • Professional experience with substance abuse, mental illness, public assistance, and homelessness. Background in homeless/low-income housing programs.
  • Ability to speak and understandSpanish.
SubstituteQualifications
  • Additional related direct service experience may be substituted for the degreerequirement.
