ResponsibilitiesProvides timely and appropriate referrals to patients in needof further medical care and services. Responds to consults,schedules procedures and handles discharge planning, asappropriate. Educates and counsels patients/familiesregarding their health care, including routine healthmaintenance, as applicable. May provide education andinstruction to residents and students.Performs or assists with procedures of higher complexity inclinic or inpatient unit. Monitors and manages invasive and/ormedical devices and placement of devices within scope ofpractice. May manage care of pre/post-operative andpre/post-procedural patients.Formulates comprehensive health management plans forhigh acuity, more complex patients in collaboration withphysicians, pharmacists, and other medical professionals toinclude progress notes, narrative discharge summaries,consults, etc. Facilitates patient participation in making healthcare decisions.Enters pertinent clinical information in the patient's medicalrecord in a timely and accurate manner and in accordancewith established compliance billing guidelines. Documentsand closes open encounters, MyChart messages, and otherEMR communications in accordance with institutionalrequirements.Call coverage may be required specific to departmentalneeds.Obtains patient histories, performs physical examinationsand/or diagnostic and therapeutic procedures, as well asorders, interprets, and evaluates diagnostic tests for patientswith high level of complexity. Reviews, prescribes and/orrecommends medications and treatments to patientsMay provide education and instruction.All other duties as assigned
