We partner with our Nurses at HCA Florida Orange Park Hospital!Job Summary and QualificationsPrimary responsibility is coordination of the Stroke Program and to ensure Primary Stroke Center, Comprehensive Stroke Center, and/or Acute Stroke Ready Designation requirements are met. Responsible for outcomes measurement for selected population and monitoring of patient/family satisfaction. The Program Coordinator is responsible for formulating short term and long term strategies to ensure the organization’s vision, mission and values are carried out through the delivery of high quality cost effective patient care for the Neurosciences service line with a high level of integrity. The Program Coordinator is also responsible for utilization, community outreach, patient advocacy, staff and physician education and marketing communications. The Program Coordinator, provides ongoing patient tracking, concurrent and retrospective chart abstracting, and data entry for performance monitoring, root cause analysis, and completion of diagnoses working with medical coders to include CPT and DRG / ICD codes based on stroke and other Neurosciences selected diagnoses. The Program Coordinator develops a long term approach with the goal to reach an uneventful and successful progression for the patient from preadmission, through the medical management and/or surgery process, inpatient stay, post-operative/discharge follow-up care, and readmission prevention to ensure the patient has a positive hospital experience and always putting health, safety and customer service of the patient first.What qualifications you will need:
(RN) Registered Nurse in the state in which you practice
Associate Degree, or Bachelors Degree, or Registered Nurse Diploma
APRN preferred
3 years Medical/Surgical Nursing required
1 year in Quality Improvement required
BLS required
NIH Stroke Scale must be obtained within 1 year of employment start date
Supervisor or Charge Nurse Experience preferred
