SUMMARY OF JOBSUMMARY OF JOB: * Provide direct patient care during daily multidisciplinary critical care rounds by collaborating with the healthcare team to optimize complex pharmacotherapy, adjust regimens and dosing based on evidence-based guidelines, and ensure safe and effective transitions of care.
  • Participate in the Critical Care Committee, contributing to the development, review, and implementation of medication-use protocols, order sets, and institutional guidelines to advance quality and patient safety.
  • Provide ongoing education to physicians, advanced practice providers, nurses, and pharmacy staff on critical care and oncology pharmacotherapy, including protocols, investigational therapies, and guideline updates, to foster a culture of continuous learning and clinical excellence.
  • Monitor and manage high-risk and specialized therapies, including vasoactive agents, anticoagulants, antimicrobials, immunotherapies, targeted oncology therapies, and investigational agents, utilizing therapeutic drug monitoring, evidence-based guidelines, and institutional protocols to optimize safety and outcomes.
  • Lead and participate in quality improvement initiatives and medication-use evaluations across critical care and oncology, such as antimicrobial stewardship efforts, adverse drug reaction monitoring, formulary reviews, and performance improvement projects to support hospital accreditation and regulatory compliance.
  • Rotate through the cancer center to provide clinical and operational support, ensuring timely verification and dispensing of chemotherapy, managing treatment modifications, responding to infusion-related reactions, and serving as an accessible clinical resource for oncology nursing staff.
  • Provide clinical coverage during evening, weekend, and on-call shifts to ensure support departmental needs including staffing support (order entry, IV room), special projects (DUE), and initiatives aligned with organizational competencies and patient population needs.
Responsible to actively coordinate, on a day-to-day basis, the provision of high quality, comprehensive pharmaceutical care to the patients of the Holy Redeemer Health System (HRHS), in compliance with legal, regulatory and professional standards. This includes drug distribution function, as well as clinical (cognitive) activities plus educational functions, all provided under the auspices of the HRHS Mission and Values.
