SUMMARY OF JOB: * Conduct comprehensive and targeted medication reviews daily to assess appropriateness, optimize therapy, and improve patient outcomes. Perform medication reconciliation at admission, transfer, and discharge to ensure accuracy, prevent discrepancies, and reduce adverse drug events.
  • Provide patient-centered counseling and education on new and existing medications, using teach-back methods to confirm understanding.
  • Collaborate with physicians, mid-level practitioners, interdisciplinary teams, and caregivers to ensure safe, effective, and coordinated medication use at the point of discharge. Identify unnecessary or inappropriate therapies and recommend initiation or discontinuation of medications prior to discharge using EMR during rounds.
  • Participate in Falls Review Committee (data collection, analytics).
  • Participate in reviewing and analyzing reports related to adverse drug reactions (ADR reporting) and medication errors.
  • Contribute to quality improvement by monitoring metrics (Quality Indicators) such as readmissions, ED visits, length of stay, medication-related problems, and patient satisfaction.
  • Collect and analyze key metrics (i.e., leapfrog) for identifying unintentional medication reconciliation discrepancies by comparing the gold standard medication history with admission and discharge orders to detect and resolve differences in medication dose, route, or frequency.
  • Participate in departmental needs including staffing support (order entry, IV room), special projects (DUE), and initiatives aligned with organizational competencies and patient population needs.
Responsible to actively coordinate, on a day-to-day basis, the provision of high quality, comprehensive pharmaceutical care to the patients of the Holy Redeemer Health System (HRHS), in compliance with legal, regulatory and professional standards. This includes drug distribution function, as well as clinical (cognitive) activities plus educational functions, all provided under the auspices of the HRHS Mission and Values.
