- Maintain a caseload of adults, youth, or families.
- Perform primary psychiatric rehabilitation including screening, intake, orientation, evaluation, referral, patient education, treatment planning, reporting and record keeping, discharge planning, and consultation with other professionals.
- Provide rehabilitation support, recovery-oriented activities, interventions, and skill development necessary for the client to improve self-management and reduce substance use, minimize psychiatric symptoms, and work on functional impairments.
- Participate in person-centered treatment planning, including risk assessment and crisis planning and assist clients to identify a meaningful life role goal and objectives through a person-centered approach.
- Provide coping skills training to improve navigation of psychiatric illness and its symptoms
- Provide social skills training, benefits and financial management, and daily living skills training.
- Assist clients with dietary and wellness education, including meal planning, healthy shopping and meal preparation and nutrition awareness.
- Support clients to increase personal autonomy, including transportation navigation.
- Provide employment services to assist individuals in pursuing, securing, and retaining employment if identified as chosen life role goal.
- Actively participate on a multidisciplinary team.
- Provide ongoing assessment of clients' progress toward recovery, functional skills and impairment levels.
- Assist with the training of staff, volunteers and student interns as assigned.
- Exhibit exemplary knowledge of the biopsychosocial process of substance use disorders/recovery, behavioral health, self-help group processes, and principles of counseling.
- Relate effectively with staff, patients, and professional community while upholding appropriate standards of confidentiality.
- Bachelor’s Degree preferred, associate's degree required with experience working with youth.
- One to three years of employment experience working with individuals with substance use and serious mental health disorders.
- An equivalent combination of training and experience may be substituted for the educational requirements.
- Proficiency with basic computer applications.
- Valid NYS Driver's License is required.
