- Evaluate hospital admissions, treatment plans, and discharge criteria based on utilization management principles.
- Review medical records to ensure compliance with insurance and regulatory standards, document accordingly.
- Conduct daily inpatient bedside rounds with a focus on discharge planning and patient education.
- Use the teach-back method to assess patient understanding of their care plan and discharge instructions.
- Prepare and deliver written discharge plans including hospitalization summary, test results, medications, follow-up care, and transportation arrangements.
- Coordinate follow-up appointments with input from patients and families.
- Conduct follow-up calls 2–3 days post-discharge to reinforce care plans and address concerns.
- Collaborate with nursing, social services, and family members for caregiving support.
- Maintain med/surg nursing competencies through annual validations.
- Identify and address barriers to care; facilitate communication among the care team.
- Support chronic care patients through education, coordination, and continued follow-up.
- Utilize the nursing process to develop and adapt treatment plans in collaboration with patients and families.
- Implement nursing care per established protocols, ensuring patient safety.
- Serve as a clinical resource and mentor to nursing staff.
- Engage in professional development, research activities, and committee participation.
- Adhere to Joint Commission standards and Welia Health policies.
- Provide age-appropriate care across the lifespan and uphold patient rights and confidentiality.
- Completes weekly trauma chart reviews
- Completes monthly data abstraction
- Attends Monthly Trauma Committee meetings
- Facilitates and presents cases at quarterly Trauma Peer Review meetings
- Attends bi-monthly ED Operations meetings in representation of the Trauma Program
- Facilitates annul hospital nursing trauma education in accordance with state Trauma Program regulatory requirements
- Facilitates annual ED Skills Trauma competency requirements
- Attends the States Trauma Program Manager meetings in the absence of the Trauma Program Manager
- Attends the Central Minnesota Regional Trauma Advisory meetings in the absence of the Trauma Program Manager.
- Collaborates with the Trauma Program Manager and Trauma Medical Director to identify gaps in quality measures, plan, implement, monitor and measure process improvement initiatives.
- Prepares monthly reports to be sumitted to the Minnesota Trauma Registry.
- This team member maintains continuous survey readiness by maintaining accurate documentation of chart audits, peer review and quality measure tracking.
