
Hospice-RN Case Manager

- RN license to practice nursing in Wisconsin upon hire; valid Wisconsin driver's license upon hire; re-certification as required.
- Minimum of 2 years public health, home health nursing or hospice experience (preferred).
- Prefer 4-year bachelor degree in nursing or 3-year diploma. Will consider 2-year associate degree with significant experience
- Possesses outstanding interpersonal skills, including excellent written and verbal skills.
- Ability to effectively work with and through physicians, subordinates, customers, and other personnel in a cooperative manner in order to improve clinical quality outcomes.
- Tactful, dependable, empathetic and courteous
- Ability to communicate in a manner that is accepted and understood by staff, residents and families.
- Ability to maintain confidentiality and professional conduct in all work-related and/or emergent situations.
- Thinks critically; utilizes sound judgment and is a team player.
- Good health and physical stamina and the ability to make in-home/facility visits.
- Experience working with computer systems; proficient in Microsoft, Excel and Word.
- Must have a valid Wisconsin driver's license and good driving record Must have auto insurance with Luther Manor minimum coverage requirements.
