- Report directly to the Executive Director or Administrator, overseeing the daily clinical operations of your assigned site.
- Ensure individualized, appropriate, and up-to-date Plans of Care for all patients, adjusting for significant changes in condition, hospital discharges, or physician updates.
- Serve as a liaison between referral sources, patients, caregivers, and your team to facilitate seamless care coordination.
- Oversee all clinical, administrative, and recordkeeping functions, ensuring compliance and accuracy.
- Provide hands-on skilled nursing care in the home when needed and participate in the on-call rotation.
- Support and guide your clinical team through coaching, performance management, and collaboration.
- Step into the site leadership role during the absence of the Executive Director or Administrator.
- Help build and maintain a positive and engaged clinical culture that prioritizes patient-centered care and regulatory excellence.
- Associate's degree in Nursing from an accredited program (bachelor's degree preferred).
- Current RN license in the state of employment.
- CPR certification required.
- Minimum 3 years of clinical nursing experience, preferably in home health or a related setting.
- Strong clinical judgment and critical thinking skills.
- Ability to lead teams, manage schedules, resolve issues, and support quality improvement.
- Proficient in nursing assessment tools and comfortable with electronic documentation systems.
- Valid driver’s license, auto insurance, and reliable transportation.
- Clinical team leadership or supervisory experience
- Home health, hospice, or case management background
- Experience navigating state and federal healthcare regulations
- Knowledge of EMRs (Homecare Homebase) and scheduling platforms
