- Support patients in the Eagle Pass area via in-person consults, telemedicine visits, and sponsored health events (i.e. pop-up clinics and community center screenings)
- Work from our brand-new Patient Support Center
- Champion a comprehensive, coordinated model of care to a mostly Medicare-aged population on a Medicare Advantage Plan
- Collaborate with our contracted/affiliated practices, to educate providers on proper coding, risk adjustment metrics, capturing documentation, and adhering to quality protocols
- Interview patients to ensure proper documentation and that all preventative screenings have been completed
- Moonlighting options available after hours or during weekends for extra income

- Base salary, plus annual quality bonus with a target of $15,000

- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current Licensure through the Texas Board of Nurse Examiners OR Licensed Physician Assistant by the Texas State Medical Board
- Valid DEA license or the ability to obtain one prior to employment
- Comfortable managing a primarily Medicare-aged patient population
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
- Valid Driver's license and reliable transportation
- Bilingual (English/Spanish) fluency
- Familiarity with primary care and chronic condition codes
- A working understanding and support of a managed care environment
