- 5+ years of outpatient pharmacy or managed care

- Ability to multi-task, prioritize essential tasks and meet deadlines
- Basic understanding of Medicare, STAR ratings
- Requires knowledge of prescription assistance programs and available resources to reduce cost of medications
- Partnership with NHMG providers and clinical staff to provide remarkable patient care.
- Assists with Medication Therapy Management and other clinical pharmacy outreach programs.
- Assists pharmacists with reviewing and processing refills, prior authorizations, and patient assistance applications.
- Obtain medication histories, including fill history, for medically complex patients.
- Addresses insurance, reimbursement, and payment issues as appropriate by communicating with insurance providers, patients, and pharmacists.
- Monitors adherence by contacting the patient at least monthly to ensure adherence and determine refill needs as applicable.
- Contacts patients and schedules appointments as necessary.
- Have a clear understanding of quality improvement principles.
- Contributes to process improvement projects.
- Ability to communicate effectively both orally and in writing while having a collaborative approach to patient care.

- Education: High School Diploma or GED, required.
- Experience: Minimum of five years experience in pharmacy, required.
- Licensure/Certification: PTCB Certification, required.
- Additional Skills (required): Considerable knowledge of medication names (both trade and generic) and their doses, routes of administration and adverse effects. Demonstrates patient interview skills and strong written and verbal communication skills with other healthcare professionals. Ability to follow verbal and written instructions. Basic technology skills. Other duties asoutlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed.
