- Receiving orders and care plans from a virtual care team to assist patients during their acute phase of illness
- Coordinating with virtual nurses and hospitalists to identify patient needs and any interventions they may require in the hospital at home setting
- Providing transition of care support and participating in home visits
- Obtaining information from patients about specific symptoms and medical questions
- Measuring and monitoring blood pressure, vital signs, and the overall condition of patients
- Providing timely and accurate reporting to the virtual care team
- Assisting patients with their activities of daily living (ADLs) such as personal hygiene, mobility, and toileting
- Offering a compassionate ear to patients and providing much-valued companionship
- Maintaining patient engagement by establishing rapport, demonstrating empathy, and building a trusting relationship
- Active, unencumbered New Jersey CNA license
- At least 1 year of experience working in an acute care, in-patient, hospital setting
- Current American Heart Association CPR for Healthcare Providers Certification
- Hepatitis B and MMR records
- Current influenza vaccination
