Specialty: Physical Therapy AssistantJob Description:Collaborates with the interdisciplinary team, the patient/family/caregiver, and the patient s primary care provider to develop a holistic plan of care focused on achieving the patient/family physical therapy goals.Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value.Promotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent physical therapy in the patient s home.Practices independently in the patient home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.Acts as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need.Assesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.Provides physical therapy treatment to the patient/family/caregiver as ordered.Completes accurate clinical documentation to effectively support service provided within specified timeframesProvides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stages.Provides instruction to the patient/caregiver and revises the treatment plan as appropriate.Maintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.Reports acute medical problems and condition changes to the primary care provider.Provides supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.Completes regulatory required assessments within specified timeframes.Completes medication reconciliation at specified timeframes.Qualifications:Must meet all state educational requirements1-2 years outpatient rehabilitation setting.At least two (2) years of physical therapy experience highly preferred.One (1) year experience in home health or other health setting with geriatric emphasis.Current license in Physical Therapy or ability to obtain prior to hire.American Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hire.Current Driver s licenseExcellent customer service skillsAbility to work without direct supervision.Provides care appropriate to the age of patients served on the job.Knowledge of physical therapy assessment and treatment methodsKnowledge of physical therapy modalities and equipmentAbility to maintain effective relationships with other health care professionalsMust be able to develop skills in use of technology used in home care setting.Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.Listed pay packages are based on weekly gross amounts.Please reference Job number: 129176
