Length of Assignment: 8-13 weeks
Hours per Week: 36 hours
Shift: 12 hour shifts, Variable - DaysJob Summary: The Licensed Practical Nurse (LPN) is a vital member of the healthcare team, responsible for delivering quality patient care under the direction of registered nurses and physicians. In this role, the LPN provides compassionate nursing care, administers medications, performs treatments, and collaborates with interdisciplinary teams to ensure the well-being and comfort of patients.Responsibilities: * Conduct nursing assessments, including vital signs, medication reconciliation, and patient history review.
  • Administer medications and treatments as prescribed by healthcare providers, adhering to medication administration policies and procedures.
  • Monitor patients' conditions, symptoms, and responses to treatment, reporting any changes or concerns to the registered nurse or attending physician.
  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and ambulation, promoting independence and dignity.
  • Document nursing assessments, interventions, and patient responses accurately and comprehensively in electronic health records (EHR) or designated documentation systems.
  • Collaborate effectively with interdisciplinary teams, including registered nurses, certified nursing assistants, therapists, and social workers, to coordinate patient care and treatment plans.
  • Provide emotional support, education, and counseling to patients and their families, addressing questions, concerns, and providing guidance on care plans and resources.
  • Adhere to infection control protocols, safety regulations, and facility policies to maintain a safe and therapeutic environment for patients, staff, and visitors.
  • Participate in educational opportunities, training sessions, and quality improvement initiatives to enhance nursing knowledge and skills.
Qualifications:
  • Graduation from an accredited Licensed Practical Nurse (LPN) program.
  • Current LPN license in Wisconsin, or eligibility to obtain licensure.
  • Minimum of 1 year of recent LPN experience in a healthcare setting preferred.
  • BLS (Basic Life Support) certification required.
  • Strong clinical assessment, communication, and interpersonal skills.
  • Ability to prioritize tasks, work independently, and collaborate effectively within interdisciplinary teams.
  • Compassionate demeanor, patience, and empathy in caring for patients.#hcc
