- Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, improve or correct disabling conditions resulting from disease or injury and help build or restore vocational/ daily living skills as well as general independence
- Provides age-appropriate education for clients and caregivers.
- Supervise and assume responsibility for care provided by identified team members.
- Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
- Provides effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
- Productive and efficient in daily operations.
- Selects and performs the appropriate assessment procedures.
- Establishes and updates formal plan of care to address individual patient needs.
- For PACE and Resource Pool, incumbents must have at least one year of experience working with the frail or elderly population.
- Must possess State License and NBCOT Certification.
- Valid NBCOT Certified Occupational Therapist - Certifications - National Board Certification In Occupational Therapy (NBCOT) and license required in state of practice.
