Position Title: Homeless Outreach Program - Housing CoordinatorGeneral Description: The Housing Coordinator provides resources to case managers in the provision of services to clients of the Homeless Outreach Program (HOP). The coordinator provides direct client services as needed, screens referrals, assigns cases, and maintains all documentation necessary to provide conformance with housing regulations. Schedule: Full time. This person reports to the Program Director.Required Education, Knowledge and Skills Abilities toPerform Essential Job Functions: A Bachelor's degree and supervisory skills are required. Experience in working with special populations is preferred. Leadership qualities are essential. The ability to work as a part of a team is necessary. A good driving record is also essential. Ability to respect individual choice and commitment to serve homeless populations is required. Also, this position requires excellent verbal and written communication skills, the ability to manage people and the understanding to respect the abilities and limitations of clients. Knowledge of Housing Quality Standards and knowledge of landlord & tenant rights is preferredSummary of Essential Functions, Responsibilities and Duties
  • Coordinate permanent housing resources for clients.
  • Ensure all documentation needed for Continuum of Care/Housing applications and placements are correct and complete.
  • Act as liaison with the Baltimore County Department of Health.
  • Assure conformance with all applicable contractual requirements.
  • Assure accurate and timely reporting of required information and statistics.
  • Ensure fiscal compliance with agency equipment, vehicles, and funds.
  • Screen referrals for housing from the Homeless Management Information System (HMIS) Coordinated Entry By Name List.
  • Report all incidents to Program Director within 24 hours.
  • Ensure client records are complete and in compliance with all required standards.
  • Provide crisis management as needed.
  • Enter client data in HMIS in compliance with grant requirements.
  • Assist with the coordination of resident moves.
  • Conduct staff selection, development, supervision and performance evaluations for Housing Case Managers.
  • Schedule yearly inspections with landlords/Case Managers/residents.
  • Perform other duties as requested by Program Director.
