- Develop and oversee individualized social service care plans, supporting residents' emotional and social well-being.
- Act as an advocate for residents, ensuring their needs and concerns are addressed and respected.
- Coordinate with healthcare providers, family members, and community resources to support comprehensive care.
- Ensure compliance with state and federal regulations and maintain accurate documentation.
- Lead and mentor the social services team, promoting a collaborative and compassionate care environment.
- Education: Bachelor's degree in Social Work or a related field.
- Experience: Prior experience as a Social Services Director or similar role in a healthcare or post-acute setting.
- Licensure: Licensed Social Worker in Alaska (or eligibility to obtain licensure).
- Compassionate Leader: Demonstrated commitment to resident-centered care and strong interpersonal skills.
- Organized and Detail-Oriented: Excellent organizational skills and the ability to manage documentation and compliance requirements effectively.
