Job Description:SCOPE: The Service Coordinator will target the delivery of community resources within communities managed by Touchstone Property Services. Coordinate and monitor supportive services and case management for individual residents and establish a comprehensive and ongoing individualized service plan that promotes wellness, personal responsibility, use of technology, exercise, activities and socialization, and community engagement, to enhance the long-term quality of life for residents. This position reports directly to the Touchstone Regional Managers.ESSENTIAL FUNCTIONS: include but are not limited to:
  • Develop contacts with community services and agency resources for resident services. Meet with service providers to promote service linkage focusing primarily on those providers that will serve the population living at the location. Coordinate information on services and assist residents, families, and staff in becoming educated in/and linked to the available services.
  • Work with local agencies, civic groups, businesses and churches to develop specific services, including working toward receiving donations from different sources for the benefit of the residents.
  • Promote volunteerism for mutual assistance, work with external organizations for the benefit of a greater community and allow for inclusiveness of volunteers for resident social events. Track and report these hours as requested.
  • Plan advocacy forum on-site annually linking residents with local political leaders or State/Federal elected officials.
  • Facilitate program and political advocacy that benefits residents (e.g. promotion of voter registration, education, etc…) at least one every quarter.
  • Promote wellness efforts for individuals and the community, and arrange health educational and disease management group programs for all residents, coordinating at least one such activity per month.
  • Encourage and support routine group activities.
  • Create and distribute a Community News Letter including but not limited to property updates, activities, agency assistance availabilities and agency changes.
  • Provide problem solving solutions through personal contact, home visits, family involvement and group meetings.
  • Encourage residents to be proactive in meeting their own social, psychological, and physical needs and being involved in decision-making process.
  • Facilitate meetings, when needed, in an acute situation and work to find more long-term solutions so as not to create dependence.
  • Securely maintain accurate, current, and complete resident files/ records in the service coordinator's office.
  • Provide reports as needed to complete organizational reporting requirements.
  • Survey residents requesting their input and ideas for education and wellness activities and topics.
  • Provide monthly outreach through the creation and distribution of newsletters and/or calendars.
  • Ensure the Resource Directory is updated on a consistent basis.
  • Make presentations to local agencies, civic groups or churches as needed.
  • Seek out additional revenue or grants available to the site for additional service related amenities or opportunities.
  • Work with residents to obtain employment in a work program or personal improvement program.
  • Complete initial Resident Intake / Assessment / Other Mental Health assessment within 30 days of move-in, if needed.
  • Work with residents in completion of all required paperwork needed for occupancy, as requested.
  • Evaluate residents annually who utilize the Service Coordinator or when a significant change occurs.
  • Perform and promote checks on resident's wellness within the building.
  • Provide monthly summary reports to supervisor of goals, objectives, and performance improvement measures being tracked for specific outcomes.
  • Regularly meet with the Site Manager and other property management staff to ensure effective team communication.
  • Required to travel between selected apartment communities to implement programs and attend meetings.
  • Contact residents absent from the Community for medical reasons at least on a bi-weekly basis to determine potential needs upon their return to the Community. Coordinate with discharge planner from the medical or other facility to have services in place whenever a resident has been absent from the facility due to rehab or extended illness.
  • Coordinate with management in evaluating ability of the resident to meet Housing Quality Standards and assist in arranging services that may be needed to meet these standards of housekeeping.
  • Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
  • Conduct monthly residential meeting with relatable topics catered to the specific assigned property.
  • Perform additional duties as assigned.
Requirements:· Bachelor's degree or the equivalent of an Associate's degree with two (2) years of college in the areas of Social Work, Social / Behavioral Sciences, Allied Health Sciences, with two years of experience working with elderly or special needs populations.· Strong writing skills, with comprehensive knowledge of composition, grammar, punctuation and spelling.· Self-motivated with the ability to work independently to complete assigned tasks.· Ability to deal appropriately with confidential information and to demonstrate a high level of professionalism.· Demonstrated ability to adhering to the code of ethics for professional social service organizations.· Demonstrated ability to exercise patience, tact and diplomacy.· Ability to interact and deal with a diverse client population.· Demonstrated proficiency in PC-based computer applications for management reporting systems, e.g. Microsoft word, excel.
