
Program Manager- Population Health-Ohio

- Lead the design, implementation, and evaluation of population health initiatives, ensuring alignment with state requirements, organizational strategies, and identified priorities.
- Build and sustain partnerships with members, community-based organizations, providers, state agencies, and other managed care organizations to advance collective health and wellness outcomes.
- Design and oversee evidence-based, and quality programs addressing behavioral, physical, and social health needs to reduce disparities and improve outcomes among high-risk sub-populations.
- Review data dashboards, analytics, HEDIS, CAHPS and other data reports to design and monitor impact and trends at the member and community level.
- Direct the development, approval, and execution of internal and external client-facing programs; manage budgets, resources, timelines, and compliance requirements.
- Contribute to the preparation, review, and timely submission of required regulatory reports; ensure program adherence to Ohio Department of Medicaid guidelines and related policies.
- Ensures program meets its stated objectives by applying quality improvement methodologies to monitor and assess program outcomes; develop success measures and conduct ongoing evaluations to ensure program effectiveness.
- Serve as the primary point of contact for external stakeholders, providing expertise, responsive solutions, and strong relationship management.
- Provides subject matter expertise in response to day to day external client facing business issues.
- Researches applicable subject matter practices and remains aware of Population Health trends, and coordinate training related to program initiatives.
- Drive engagement across departments to support enterprise-wide participation and ensure program objectives are met.
- Perform other related additional responsibilities as assigned.
- Requires a BA/BS and minimum of 5 years external client facing experience in program/project management; or any combination of education and experience, which would provide an equivalent background.
- BA/BS degree in Public Health, Health Administration, Business Administration or related field is strongly preferred.
- Graduate degree preferred.
- Project management certification preferred.
- Minimum of 5 years of program/project management experience in a healthcare or public health setting with direct client-facing responsibilities strongly preferred.
- Experience in in managing population health initiatives and delivering measurable, data-driven outcomes strongly preferred.
- Familiarity with the Ohio Department of Medicaid’s Population Health Stair-Step approach strongly preferred.
- Experience in regulatory compliance, mandatory reporting, and meeting strict deadlines preferred.
- Proficiency in quality improvement techniques and methodologies strongly preferred.
