This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace. When reporting to the office, the office assigned is OH-COLUMBUS, 8940 LYRA DR, STE 300. Alternate Office locations within Ohio may be considered if candidates reside within a commuting distance from an Ohio office. Travel within the state of Ohio as needed may be required.*Please note that per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law.Schedule: This position will work a first shift from 8:00 am- 5:00 pm EST Monday thru Friday.The Program Manager- Population Health, is responsible for the development, implementation, evaluation and ongoing management of multi-year strategies designed to improve health outcomes and advance wellness across Ohio. This role oversees an integrated portfolio of internal and external population health initiatives, ensuring alignment with the Ohio Department of Medicaid priorities and state-designated population health goals. The Program Manager provides strategic direction, performance oversight, and subject matter expertise to drive measurable improvements in member health outcomes and community wellness, particularly among sub-populations with identified health-related social needs.Primary duties may include, but are not limited to:
  • Lead the design, implementation, and evaluation of population health initiatives, ensuring alignment with state requirements, organizational strategies, and identified priorities.
  • Build and sustain partnerships with members, community-based organizations, providers, state agencies, and other managed care organizations to advance collective health and wellness outcomes.
  • Design and oversee evidence-based, and quality programs addressing behavioral, physical, and social health needs to reduce disparities and improve outcomes among high-risk sub-populations.
  • Review data dashboards, analytics, HEDIS, CAHPS and other data reports to design and monitor impact and trends at the member and community level.
  • Direct the development, approval, and execution of internal and external client-facing programs; manage budgets, resources, timelines, and compliance requirements.
  • Contribute to the preparation, review, and timely submission of required regulatory reports; ensure program adherence to Ohio Department of Medicaid guidelines and related policies.
  • Ensures program meets its stated objectives by applying quality improvement methodologies to monitor and assess program outcomes; develop success measures and conduct ongoing evaluations to ensure program effectiveness.
  • Serve as the primary point of contact for external stakeholders, providing expertise, responsive solutions, and strong relationship management.
  • Provides subject matter expertise in response to day to day external client facing business issues.
  • Researches applicable subject matter practices and remains aware of Population Health trends, and coordinate training related to program initiatives.
  • Drive engagement across departments to support enterprise-wide participation and ensure program objectives are met.
  • Perform other related additional responsibilities as assigned.
Minimum Requirements:
  • Requires a BA/BS and minimum of 5 years external client facing experience in program/project management; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
  • BA/BS degree in Public Health, Health Administration, Business Administration or related field is strongly preferred.
  • Graduate degree preferred.
  • Project management certification preferred.
  • Minimum of 5 years of program/project management experience in a healthcare or public health setting with direct client-facing responsibilities strongly preferred.
  • Experience in in managing population health initiatives and delivering measurable, data-driven outcomes strongly preferred.
  • Familiarity with the Ohio Department of Medicaid’s Population Health Stair-Step approach strongly preferred.
  • Experience in regulatory compliance, mandatory reporting, and meeting strict deadlines preferred.
  • Proficiency in quality improvement techniques and methodologies strongly preferred.
