Location: Hanford Post Acute – Hanford, CA
Rate: Starting at $60/hour (Based on Experience)Hanford Post Acute is seeking a Part-Time Physical Therapist to join our dedicated rehabilitation team. This role is ideal for a clinically skilled PT who is passionate about delivering high-quality therapy services in a post-acute care setting.Responsibilities:Evaluate patients within 24 hours or 1 business day of physician referral; document cause for any delayDevelop individualized treatment plans and obtain physician approvalDeliver care as outlined in the treatment planSupervise Physical Therapy Assistants in patient care and related activities in compliance with the state practice actCommunicate with interdisciplinary team regarding patient progress and care planningParticipate in patient care conferences, utilization reviews, and in-service trainingMaintain accurate and timely documentation of evaluations, daily notes, progress updates, and recertifications in accordance with PT Board and payer requirementsInstruct caregivers and nursing staff on maintenance programs and post-discharge careRecommend and assist in ordering durable medical equipment to enhance patient mobility and independenceReport and monitor maintenance needs of therapy equipmentSupport nursing with Restorative Aide trainingSupervisory Responsibilities:Supervise PT assistants, aides, and students as applicableQualifications:Bachelor’s degree in Physical Therapy required; Master’s or Doctorate preferredCurrent California PT license in good standingCPR certification and ongoing CEUs as required by the stateStrong documentation skills, preferably with experience in Casamba and PointClickCareExperience in SNF or post-acute care preferredAbility to work collaboratively and maintain positive relationships with residents, families, and staffPhysical Demands:Frequent standing, walking, reaching, and lifting (up to 100 lbs occasionally)Occasional stooping, kneeling, or crawlingVision requirements include close, color, peripheral, and depth perceptionWork Environment:Low to moderate noise level typical of a clinical settingReasonable accommodations available as neededApply Now:
