Join us on this meaningful journey where your skills, compassion and dedication will make a remarkable difference in the lives of those we serve.How you'll contributeA Social Worker (LSW) who excels in this role:Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.Assists with discharge planning and processes. Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state (CSW, LPCA, LCSW) within 6 months of hire.Certifications: Handle With Care within 30 days of hire.Connect with a Recruiter
