The Psychiatric Nurse Practitioner will provide general outpatient psychiatric services to members of our onsite community, including psychiatric intakes and evaluations for acute and chronic conditions along with related treatments, medication evaluations and treatment option planning. This role will work closely with our primary and ancillary care teams, including other behavioral health providers, as part of our larger comprehensive primary health model.Job ResponsibilitiesAssess and treat acute and chronic psychiatric conditions through a combination of psychotherapy, psychoanalysis and medication management; discussing progress toward goals with patients, providing instructions, monitoring effect of medication(s), with referral for inpatient services as needed.Develop individualized care and treatment plans by determining the nature and extent of cognitive, emotional, developmental, social, and behavioral disorders, and establishing treatment goals and methodologies, collaborating with behavioral health providers as appropriate.Engage in regular discussions with patients to change behavioral patterns, explore past experiences, and/or conduct group and family therapy sessions.Provides treatment for acute and chronic psychiatric conditionsCompletes medication reviews and prescribes medicationsAdvise or inform guardians, relatives, or significant others of patients' conditions or treatment, as appropriate.Provides evidence-based, timely, compassionate careWorks with a multidisciplinary medical team, including primary care providers, health coaches and other behavioral health specialistsComplete thorough documentation using EMR and other integrated tools and applications, collecting medical information from patients, family members, and other medical professionals.Actively participates in quality improvement programs, as well as in multidisciplinary quality assurance programsCollaborates with the management team as needed to ensure improvement of patient careCooperates with clinical outcome reviews and measurementsComply with all Crossover Health policies, procedures, and protocolsPerforms other duties as assigned.Required QualificationsNP degree from an accredited programActive Washington State Medical LicenseCompletion of accredited Psychiatric-Mental Health Nurse Practitioner (PMHNP) Post-Master’s certificate programAmerican Nurses Credentialing Center (ANCC) Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification.Minimum 5 years of clinical experience serving patients across the care continuum of psychiatric careCurrent BLS (Basic Life Support) certificationPreferred QualificationsExperience in an outpatient setting, specifically within an integrated primary care modelExcellent communication skills both verbal and written, including presentation and consultation skills.Experience with telehealth or virtual care offerings, including aptitude for associated technology.Competent using computer applications including database, spreadsheet, and document creationSelf-directed individual with polished professionalism and the ability to prioritize multiple tasksAbility to use discretion and judgment in dealing with sensitive or confidential informationPassion for best-in-class member experiences (patient-centered medical home model) and for providing next generation health careThrives in a fast-paced and rapidly changing, demanding environment.Experience using EMR systemsPhysical Job RequirementsMay require standing, walking and sitting for extended amounts of time.Occasionally lift and carry items weighing up to 50 lbs.Manual and finger dexterity and hand-eye coordinationIncludes full range of body motion including potential of handling and lifting patients.Requires corrected vision, hearing and speech within normal ranges. Must be able to effectively communicate with patients and team members.The base pay range for this position is $65.54 to $75.38 per hour. Pay range may vary depending on work location, applicable knowledge, skills, and experience.
