- Supervise, teach, and coach Direct Support Professionals and Lead Direct Support Professionals according to DHS licensing requirements and PAI performance standards. Ensure professional and regular communication to ensure ideas, concerns, and recommendations are given due consideration.
- Train and supervise program staff to 245D requirements and PAI policies and procedures.
- Provide supervision, support, evaluation, and development of participant's needs as specified in 245D licensing standards, PAI policies, and other applicable rules and regulations.
- Coordinate interviewing, hiring, on-boarding, and retention of staff.
- Oversee and author participant Coordinated Service and Support Plan Addendum (CSSPA), Individual Abuse Prevention Plan (IAPP), Self-Management Assessment (SMA), person-centered outcomes, and provide direct observation of staff members to ensure effectiveness of service delivery.
- Respond to identified support needs, interests, preferences, and desired outcomes for each participant.
- Oversee program staff to ensure adequate daily staffing.
- Ensure participant personal care needs are met and that proper transfer/lifting procedures are followed.
- Meet Designated Coordinator standards as established per 245D. Minimally have one of the following:
- A Baccalaureate Degree in a field related to Human Services and one-year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older;
- An Associate Degree in a field related to Human Services and two-years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older;
- A diploma in a field related to Human Services from an accredited post-secondary institution and three-years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older; or
- Minimum of 50-hours of education and training related to Human Services and disabilities, and four-years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older under the supervision of a staff person who meets one of the qualifications in a-c above.
- 21+ years of age or older.
- Pass an OIG and DHS background study and maintain approved status without continuous supervision.
- Reliable transportation for work driving purposes, a valid driver's license, proof of valid car insurance, ability to satisfy insurance criteria for driving history and permit driving record checks as needed.
- Experience with and understanding of Minnesota Statutes Chapters 245A, 245C, and 245D.
- Experience with program development, training, and quality assurance or any equivalent combination of experience and training that provides the required knowledge, skills, and abilities.
- Leadership and/or management experience.
- Experience in a Day Training and Habilitation program or other disability services.
