- Visits with assigned caseload of patients and families in their homes to complete guided assessments, support medication management, address complex symptoms and coordinate care.
- Engages patients and caregivers in discussions about their goals of care, review advance directives and collaboratively develop treatment plans based on goals.
- Monitors care plans in conjunction with the rest of the clinical care team.
- Advocates for the patient and caregivers in liaison with other care providers to ensure care is coordinated and consistent with patient's goals.
- Participates in the measurement and assessment of care-related processes and outcomes to improve the quality of patient care.
- Communicates plans of care to primary physicians and other consulting clinicians.
- Educates patients and family on trajectory of illness, purpose of medications (also reviews side effects and how to administer) and assesses disease understanding.
- Performs other duties as assigned.
- Associate degree in nursing (bachelor's preferred).
- Current RN license, specific to the state(s) you are assigned to work.
