
LPN/RN - Primary Outpatient Care (Float) (Rockland and Westchester)

- Conduct patient interviews to gather health history, including current complaints and medication use, using patient identifiers. Document findings in charts with time stamps or initials.
- Maintain medical records, noting nursing observations, vital signs, test reactions (e.g., PPD results), and relevant information.
- Prepare equipment and assist the physician during patient examinations and treatments.
- Initiate necessary follow-up actions and provide patients with healthcare instructions as needed.
- Assess and triage patients in person or via telephone.
- Demonstrate knowledge of growth and development principles to provide appropriate care.
- Stay updated on nursing procedures through mandated training.
- Maintain patient records, including history, vital signs, test reactions, and laboratory results, ensuring completion of Clinical Visit Records (CVRs) as needed.
- Perform in-house labs, collect specimens for outside labs, and conduct quality control according to protocols.
- Complete assigned tasks and logs and follow up on actions as directed by the provider.
- Provide health education as indicated, referring patients to a Registered Nurse when necessary.
- Collect patient data under the supervision of a Registered Nurse or provider, without independently interpreting clinical data.
- Minimum Education Requirement: High school diploma/ GED
- Preferred Education Requirement: Vocational/ Technical School/ Diploma Program - Nursing
- Minimum Work-Related Experience: 0-1 year
- Preferred Work-Related Experience: 2-4 years
- Minimum Licensure:
- Valid New York State LPN License
- CPR Certification
- Infection Control Certification
- Minimum Education Requirement: Bachelor's degree
- Minimum Work-Related Experience: 1 - 4 years of experience
