- Provide clinical oversight and supervision to all home health nursing and therapy staff and hospice clinical teams.
- Ensure timely and accurate completion of assessments, care plans, documentation and visit notes.
- Review and approve clinical documentation for quality, completeness, and compliance with state, federal, and accreditation standards.
- Coordinate patient care and ensure continuity and quality of services.
- Ensures necessary equipment, supplies, and staffing are available for the delivery of the prescribed patient care
- Monitor caseloads, productivity, and visit frequencies to ensure efficient and effective patient care delivery.
- Assist in staff recruitment, onboarding, training, performance evaluations, and ongoing education.
- Conduct case conferences, team meetings, and performance improvement initiatives.
- Respond to and assist in investigation of patient concerns or clinical incidents.
- Report all potential complaints and/or grievances to the Executive Director.
- Collaborate with physicians and other healthcare providers to ensure appropriate care planning.
- Participate in audits, surveys, and other regulatory or accreditation processes.
- Maintain up-to-date- knowledge of Medicare Conditions of Participation, state regulations, and best practices in home health and hospice.
- Maintains confidentiality of patient information, following agency policies and procedures implementing HIPAA requirements for the privacy and security of protected health information. Uses and/or discloses only minimum amount of Protected Health Information necessary to complete assigned tasks and reports all suspected violations of HIPPA policy to the Senior Clinical Director.
- Must be available for significant travel throughout the state of Wisconsin
- Completes an initial physical evaluation of patient as prescribed by the attending physician.
- Submits clear and appropriate documentation in a timely manner and documents patient education in the medical record.
- Communicates effectively with physicians, supervisors, and coworkers in a timely manner.
- Develops a care plan, which establishes goals based on diagnoses and patient and/or caregiver specific goals. Includes the patient and the family in the planning process and adapts the plan of care as patient needs change.
- Complies with agency policies and procedures. Promotes and maintains an agency environment in compliance with federal, state, and local regulations.
- Counsels the patient and family in meeting related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Registered nurse with a current, unencumbered license in WI; Bachelor’s degree in nursing preferred.
- Current licensure in state and CPR certification.
- Two (2) years of home health care and/or hospice experience preferred, including at least 1 year in a supervisory or leadership role.
- Excellent observation, verbal and written communication skills, problem solving skills, leadership and interpersonal skills, and ability to manage multiple priorities.
- Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
