Cornell Scott-Hill Health Center is seeking a full-time Care Coordinator to support the Judicial Branch Court Support Services Division!The Care Coordinator position will directly support the Judicial Branch Court Support Services Division’s (JBCSSD) Adult Behavioral Health Services (ABHS) program at CS-HHC. The Care Coordinator will serve as the liaison between JBCSSD, ABHS and CSHHC. The Care Coordinator will assist the ABHS team, JDCSSD and CSHHC behavioral health team through direct and indirect response to the behavioral health care management issues and needs of CS-HHC patients/clients referred by ABHS/JBCSSD. Care coordinator responsibilities will include the engagement and support of individuals as potential participants from the beginning of the referral process, throughout their treatment and completion of services including community-based engagement and follow up. The Care Coordinator will be responsible for the input of client level data into the Contractor Data Collection System (CDCS); maintenance of timely and accurate progress and court reports; and other support services to care management team as needed.Care Coordination will include assisting Behavioral Health providers with clinical support services and data management. The Care Coordinator will assist in helping referred clients navigate the health care system and obtain appropriate medical and dental services at CS-HHC. The Care Coordinator will be responsible of facilitating communication between JBCSSD and ABHS. The Care Coordinator will focus on increasing the number of clients connected to either public or private insurance; entitlement assistance to obtain Medicaid/Medicare and DSS application process.Responsibilities include but are not limited to:
  • Receives JBCSSD referrals, assesses appropriateness for care.
  • Works to provide and improve patient continuity before, during, and following engagement within the health system.
  • Assists in the development of a comprehensive complex care management plan, with particular emphasis on socially or psychiatrically complex issues.
  • Facilitate communication between CS-HHC, JBCSSD and ABHS
  • Compliance Work Plan that identifies potential compliance risk areas as well as incorporates initiatives outlined in the OIG Work Plan.
  • Coordinates care with other members of the care team to overcome any identified barriers that inhibit patients’ care.
  • Records referral follow-up activity, progress and outcomes in the medical record.
  • Outreach and engagement in the community of assigned JDCSSD clients as needed to ensure access to services and follow as appropriate.
  • Assists client and care management team in obtaining and maintaining insurance, entitlement and other resource authorizations as needed to provide core health and behavioral health services and referral services.
  • Educates patients regarding how to navigate throughout the health system.
  • Performs all other duties as assigned by the Program Director or designee.
Requirements:
  • Master’s Degree in Behavioral Health field is desirable, Bachelor’s Degree or higher required.
  • Minimum of one (1) year experience working with relevant population required (internship may qualify as experience).
  • Knowledge and competency regarding diagnosis, assessment and treatment of behavioral health services.
  • Ability to analyze claims, encounters, billing, or other data to identify deficiencies; compile objective data, produce written reports, and develops corrective action plans.
  • Ability to generate high levels of client satisfaction
  • Strong written and verbal communication skills along with excellent follow-up skills.
  • Ability to use computer and telephone; experience using an electronic health record is desirable
  • Must demonstrate age specific/population specific competencies in area(s) of practice Strong orientation to deadlines and details.
