2. Medication/Solution Order Processing - Utilizes the full functionality, which includes allergy review, duplicate therapy, drug-drug interaction, dosage range, IV incompatibility and food-drug interaction screening of the automated order entry system to facilitate accurate and timely medication/solution distribution and administration. (Pharmacy Information System Competency) Guide - Utilization of the Pharmacy Order Entry System. Order entry processing transactions (includesnew, discontinued, update for IT, IV, and MED). Transactions based upon a monthly total reviewed fornumber of days worked in the calendar month. Quantifiable with HMM order entry statistics.
3. Intervention Documentation – Documents interventions using the approved documentation tools. (Includes Safety-First Reporting, Medication Errors/ Near-miss Reporting) Guide - # Interventions and Cost avoidance Greater than 2500 interventions and cost avoidance $187,500 or greater per year = Far Exceeds 2000 - 2499 interventions and cost avoidance $150,000 - $187,499 per year = Exceeds 1500 - 1999 interventions and cost avoidance $112,500 - $149,.999 per year = Meets 4. Technician and ancillary staff supervision- Reviews medications prepared for distribution by technicians with respect to accuracy, appropriateness, and timeliness. Monitors technician accountability to follow established policies and procedures for medication delivery, including the automated dispensing system. Reviews technician preparation of intravenous admixtures, parenteral nutrition compounding, chemotherapy and miscellaneous sterile products.
5. Drug Information Resource – Provides pertinent drug information to medical and ancillary hospital staff. Researches medication dosages, indications, interactions, compatibility, and administration when requested by healthcare professionals. Responds timely and in a professional manner.
6. Renal Dosing Support - Reviews prescriber's orders for appropriate drug therapy relative to patient's renal status (CrCl is calculated accurately, drugs requiring adjustment are identified, other clinical factors are considered when dosage adjustments are made). Participates in the application of renal dose adjustments for medications identified in the Bayhealth Renal Dose Policy, B7700.72
7. Departmental Performance Improvement Participation – Participates in departmental performance improvement program by ensuring that all quality control measures are followed and properly documented. This includes but is not limited to: Monitors for Automated Distribution Cabinet Report Reviews, e.g. Override and Discrepancies; Monthly Narcotic Inspections. Includes documentation requests for technician near-misses and identification of medication use scenarios which require submission of Safety First Reports.
8. Education Resource – Conducts staff development programs and provide educational service to pharmacy staff, pharmacy students and residents; nursing staff, physicians, and other healthcare professionals.
9. Performs other duties as assigned in accordance with Bayhealth Standards. Maintains current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances and investigational agents; conducts personal practice in accordance with those requirements.Required Education, Credential(s) and Experience:
