From routine care to the most complex cases, pediatricians and families trust us to provide the most advanced medical and surgical care for kids.ResponsibilitiesProvides key social work services in a variety of pediatric settings across the health care continuum. Key services include (but are not limited to): psychosocial assessments, crisis management, planning and coordination of internal and external services, resource connection, transition/discharge planning, supportive counseling, psychosocial education, post-discharge follow-up, and in limited settings, concurrent utilization review. This position is intended for use in smaller operating units, as part of a resource pool, or for occasions that call for sporadic or stand-by work scheduling only.QualificationsEducation:Masters degree in social work from an accredited school of social work.Experience:Minimum one year post-Masters experience in one or more of the following areas required: Healthcare, discharge planning, case management or coordination of home based services, or has previously completed a Legacy Social Work internship. Pediatric medical experience strongly preferred.Skill Requirements:
  • Broad knowledge of the principles of social work and the ability to apply them
  • Demonstrates strong organizational skills, including an ability to effectively triage patient and family needs
  • Practices with strengths based practice focus
  • Engages in culturally competent practice
  • Demonstrates excellent verbal and written communication skills for effective and appropriate interactions with patients, families, physicians and staff
  • Effectively engages in discharge/transition planning
  • Demonstrates ability to locate, utilize and advocate for patient/family access to hospital/ community resources
  • Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues

  • Licensed Clinical Social Worker (LCSW) or Clinical Social Work Associate (CSWA, working toward LCSW) required for employees who perform this job in the state of Oregon.
  • LCSW eligible strongly preferred for employees who perform this job in the state of Washington.
  • Washington Counselor registration required for state of Washington employees who do not currently have a LCSW.
