We are currently seeking an RN Case Manager in Southern Fairfield County. The RN's overall responsibility is to provide direct patient care and case management to an assigned caseload in the home setting in accordance with physician's orders. This includes admission of patients for service and determining and developing a plan of care. This position will cover towns in Fairfield County region, such as Wilton, Weston and Redding, CT.Job Responsibilities:
  • Provides direct patient care and case management. Competently implements 24-hour responsibility for each patient including nursing and home health aide service.
  • Identifies and documents patient and family needs through comprehensive assessment. Reports all findings to M.D., coordinates services, and follows through on Physician orders.
  • Performs admission procedure as per RVNAhealth policies using legible, accurate documentation. Develops a plan of care consistent with the physician's plan of treatment.
  • Performs evaluation of patients' progress, promptly acts on any changes made to patient's plan of treatment or condition, including appropriate discharge procedure when goals have been met.
  • Updates and maintains the nursing care plan. Documents communication and interprets plan of care to the patient regarding diagnosis and treatment.
Qualifications/Expectations
  • Graduation from an accredited School of Nursing.
  • Currently licensed to practice as an RN in the State of Connecticut.
  • Has a minimum of one year of nursing experience.
  • Ability and willingness to follow Agency policies and procedures and to work as a team member supporting the agency mission, vision, and values.

This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting.
