PURPOSE OF POSITIONThe Clinical Resource Nurse is responsible for clinical triage from patients, families, physician, coordinating clinicians and another other outside agencies. The purpose of the position is to manage patient care from phone calls, emails, or other lines of communication in accordance with the best practice standards and regulations so that the highest degree of quality, responsiveness and compliant care is maintained. Utilizes and promotes use of excellent customer service skills when working with patients, families, physicians, and staff.RESPONSIBILITIES & EXPECTATIONS
First contact for clinical calls from patients, physician offices and clinicians
Works collaboratively with Clinical Managers, team assistants and others to problem solve and implement orders
Provides patient/family/caregivers with education and information pertinent to the diagnosis and plan of care; counsels and educates patients and caregivers
Participates in coordination of home health services with all physicians involved in the plan of care, other health care practitioners as appropriate, other agency disciplines, and other internal and external stakeholders to promote continuity of care
Communicates appropriate information to after hours and other staff to ensure patient status is coordinated
Completes documentation of all calls and patient care activity in the clinical record
Reviews faxed orders and SBARs; enters new and/or changed orders into the medical record. Ensures that the case manager, Clinical Manager and others providing care are notified of new and changed orders in order to ensure appropriate care.
Receives and documents telephone orders. Updates the record as required with new/changed orders
Provides direct patient care in accordance with the plan of care and physician orders as needed
Reviews physician orders and follows up with patient care for clarifications or additional information if needed
Identifies objections, concerns or other entry barriers and provides information in response to address patient care issues.
Maintains positive professional relations with support personnel through appropriate and timely response for information or paperwork necessary
Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community
Demonstrates flexibility in meeting the needs of the department and responds positively to changes in workflow
Promotes teamwork by demonstrating willingness to assist others in meeting the needs of all individuals and collaborating effectively with the interdisciplinary team and other departments throughout the organization
Complies with agency infection prevention and control and safety policies
Completes annual competency requirements
Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel
Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Director of QAPI
Consistently maintains a professional appearance
Protects patient health information according to the HIPPA rule and related Aultman policies

Must be a registered nurse with a minimum of six months’ recent clinical experience, minimum of one year of recent clinical experience preferred
Experience with at least one year’s experience in a home health, hospice, or community heath setting strongly preferred
Clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations
Exceptional interpersonal and customer service skills; must be able to effectively solve unique problems as they arise or identify when to consult the supervisor.
Knowledge of current home health regulations and Medicare, Medicaid, State and Federal regulations
Demonstrated problem-solving abilities and excellent telephone, interpersonal and written communication skills.
Strong commitment to teamwork and customer service, both internally and externally, is required
Strong/proven data entry and general computer skills
WORKING CONDITIONS:
Hours of operation with shifts as assigned including occasional overtime, on-call or off-shifts (evenings and/or weekends) scheduled as necessary.
Subject to frequent interruptions and changes in priority of duties throughout the day.
May be subject to emergency and other crisis situations
