- Provide direct patient care under the supervision of an RN.
- Monitor resident vitals, administer medications, and assist with treatments.
- Assist with daily living activities and resident care as needed.
- Work collaboratively with nursing leadership and community interdisciplinary team to communicate resident progress and concerns.
- Document resident care accurately and maintain electronic medical records.
- Maintain a safe, clean, and organized environment for colleagues, residents, and visitors.
- Act as a liaison between the nursing team, providers, and residents and their family members.
- Ensure compliance with regulatory, safety, and infection control standards and protocols.
- A current LPN license in New York State.
- The ability to remain calm and effective under pressure.
- Previous experience in long term care preferred.
- Strong communication and interpersonal skills.
- The ability to work effectively in a team environment.
- Time management skills.
