Compensation & Benefits
Salary: $110,000 – $125,000 per year (based on experience + achievable monthly bonuses)

Key Responsibilities
Conduct comprehensive patient assessments and create individualized care plans
Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
Collaborate with interdisciplinary facility teams to ensure coordinated care
Educate patients, families, and staff on health management and treatment plans
Complete documentation within 24 hours
Ensure compliance with all applicable state and facility regulations
QualificationsQualifications * Active Nurse Practitioner (NP) License in state of practice.
2 years of experience preferred but not required
Preferred experience in:
Skilled Nursing Facilities (SNFs)
Hospice and/or palliative care
Emergency department, ICU/SICU, or hospitalist settings
Proficiency in Electronic Health Records (EHR)
Strong communication skills and a collaborative, compassionate mindset
