
Travel Home Health LPN

- One year of recent experience as an LPN, including a minimum of six months in a Home Health setting as an LPN
- Valid LPN license in compliance with state regulations
- Current BLS (AHA/ARC) Certification
- Other certifications and licenses may be required for this position
- Deliver direct patient care in accordance with individualized care plans, physician orders, and nursing protocols
- Administer medications and treatments, including injections, wound care, and monitoring for side effects
- Assist in educating patients and caregivers on proper medication use, treatment plans, and home care techniques
- Observe and report changes in patient conditions to the supervising RN or physician for timely intervention
- Collaborate with RNs, therapists, and other healthcare providers to ensure seamless, high-quality patient care
- Maintain accurate and timely documentation of all care provided, patient responses, and any concerns
- Ensure compliance with infection control, safety, and home care best practices to protect patients and caregivers
- Support families and caregivers, offering guidance and emotional support throughout the care process
- Maintain communication with case managers and interdisciplinary teams to provide coordinated patient care
- Strong ability to complete charting and documentation requirements while meeting visit and productivity goals
- Adhere to infection control, safety, and healthcare regulations
- Perform other duties as assigned within the scope of practice
- Critical thinking, service excellence and good interpersonal communication skills
- The ability to read, write, & communicate in the English language is required
- Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
- Physical Abilities - Remain in a stationary position, move about, move equipment (50-100lbs), pushing, pulling, bending
- Interpersonal Skills - To work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
- Technical/Motor Skills - Ability to grasp, fine manipulation, pushing/pulling and able to move about when assisting with procedures and/or using department equipment.
- Mental Requirements - Must be able to cope with frequent contact with the general public, customers, and meeting deadlines under pressure. Must be able to work under close supervision occasionally as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail
