Impact Mental Health has full-time openings for Connecticut licensed APRNs to provide psychiatric consultation evaluations supported by our team of APRNs, LCSWs, psychologists, and MDs.While providing a strong team atmosphere, Impact allows its providers to practice with autonomy and independence. We provide a full benefit package with a competitive salary and room to grow. Providers are allowed to create a schedule that works best for them and their assigned buildings within typical business hours, 8:00am - 5:00pm.Responsibilities:
Provide person-centered care using best practices
Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
Work within a team of social workers, therapists, nurses, and physicians to coordinate care within Impact and the skilled nursing facility
Provide collaboration with medical team as needed
Submit required documentation within 24 hours of encounter
Adhere to state regulations within SNF setting
Provide assessment, diagnostic, and treatment services
Provide psychoeducation and medication education to patients and/or families
Experience:
Experience in skilled nursing facilities, hospitals, residential care, geriatric care, and psychiatric outpatient preferred

Family, Geriatric, or Psychiatric Nurse Practitioner with active license in Connecticut
2+ years clinical experience managing mental health care for adults
Experience in Skilled Nursing Facilities and/or geriatric population mental health care preferred
Strong communication, assessment, diagnostic, and treatment skills
