Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.Job Summary:The Nurse Navigator serves as a point of contact for the referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered by EvergreenHealth and in the community. Facilitates patient appointments and procedure scheduling, including labs, diagnostic areas, and specialty physicians to ensure all patients receive quality and comprehensive services. Functions on the multidisciplinary team as an advocate, educator, and counselor for Cardiology patients, especially those who need cardiology procedures.Primary Duties:
  • Serves as an essential link between Cardiology patients and all other care providers, while patient moving through healthcare system from in office visits, virtual communication of the care, and follow up after procedures.
  • Assists patients in understanding their diagnosis, treatment options, and the resources available before and after their procedures.
  • Educates patients and family about the procedures and any other related resources for clear understanding.
  • Follows patients throughout the care continuum, including partner with procedure scheduler in coordinating pre procedure testing, and collaborates with inpatient, outpatient, and community care management resources.
  • Responsible for the outreach efforts to establish and maintain positive working relationships with key customers (physicians, mid-levels, office staff, diagnostic staff, nurses, and other care team members).
  • Performs other duties as assigned.
License, Certification, Education or Experience:REQUIRED for the position:
  • Bachelor's in Nursing (BSN)
  • Licensed as a Registered Nurse in the State of Washington
  • 5 years of clinical Cardiology experience
  • Current Healthcare Provider BLS certification by date of hire
