Job Summary:Responsible for the delivery of direct, hands-on care of clients in homes or facilities. Responsible for assessing clients, documenting observation and assessment data, evaluating outcomes, and implementing the nursing plan of care.Essential Duties and Responsibilities:· Implements the medical plan of treatment and follows physicians' orders.· Monitors individual at all times, following all CHC and agency safety protocols.· Transports clients to activities, services, appointments, and general community per ISP, HAH and agency.· Documents activities and services accurately and timely according to CHC and agency regulations.· Monitors general health conditions of the individual and notifies supervisor of any medical issues or if treatment is needed.· May provide skilled care (G-Tube, meds, etc.) to individuals based on the individual’s care plan.This description reflects assignment of essential functions, management may assign or reassign duties and responsibilities to this job at any time that are not listed above.Required Skills/Abilities:· Ability to work in a professional, ethical, and respectful manner.· Ability to provide personal care for non-ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to community events and social activities.· Strong communication skills, oral and written; strong interpersonal skills.· Ability to anticipate and resolve problems demonstrating good judgment.Education and Experience:· Current RN licenses.· Current CPR/First Aide certification.· Current TB screening.· Minimum three (3) years of experience providing skilled care to the medically fragile.· May be required to have current certification in Advanced Cardiac Life Support (ACLS) and/or Pediatric Life Support (PALS).Management Authority:☐ Makes staffing decisions – hiring, firing ☒ Trains other associates☐ Conducts performance reviews ☐ Directs work of other associates☐ Creates and directs strategic goals ☐ Direct workflows for entire teamPhysical Requirements:☐ Sedentary – ability to remain in a stationary position for extended periods of time.☒ Ability to move, transport, or position: ☒ up to 50 pounds; ☐ up to 100 pounds☒ Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.☒ Ability to communicate effectively and clearly with others to exchange information.Travel Requirements:☒ Regular travel on a daily or weekly basis required, even in inclement weather☐ Monthly or quarterly travel required☐ Little to no travel requiredThe above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
