- Perform administrative duties per business processes and timelines including fax, telephone outreaches to members, providers, data entry and perform follow-up tasks
- Identify, initiate and process referrals for eligible programs and services available to our members timely
- Collaborate with staff, medical providers and therapy agencies to provide high quality service coordination to our members
- Cross train with other departments within SK program to provide coverage and assistance as needed
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
- Performs all other related duties as assigned

- High School Diploma/GED (or higher)
- 2+ years of clerical or administrative work experience
- 1+ years of experience with knowledge of the resources available, culture, and values in the community
- Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and be able to use multiple web applications
- Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Ability to travel up to 10% of the time for team meetings or company events
- Valid/Current Community Health Worker Certification by the State of Texas
- Associate's Degree
- Experience working with the Medicare and Medicaid populations
- Experience with electronic charting
- Experience with arranging community resources
- Bilingual - Spanish and English
- Background in managing populations with complex medical or behavioral needs
- Knowledge of Center for Medicare and Medicaid Services and Regulations
- Proven ability to interact productively with individuals and with multidisciplinary teams
- Proven excellent organizational and prioritization skills
- Works with others as part of a team
