
Medical Social Worker

- Complex discharge planning based on Continuity of Care assessment and anticipated post-acute care needs in direct consultation with patient, family and multidisciplinary team which incorporates choice
- Provides counseling services to patients and their families to assist with adjustment to illness and /or disability with regards to the patients cultural, religious and ethnic background and needs
- Provides consultation to the Health Care team to establish and implement appropriate treatment goals and discharge plans with regards to the patients cultural, religious and ethnic background and needs
- Interacts with outside agencies on issues of suspected abuse/neglect/domestic violence/guardianship and other social/legal matters
- Uses knowledge of insurance benefits and coverage guidelines to maximize resources
- Delivers CMS regulatory notices within CMS established timeframes
- Educates staff, patients and families about available community resources
- Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
- Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct; maintains licensure and abides by NASW standards
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and Masters of Social Work (MSW) required
- Or Licensed Clinical Professional Counselor and Masters of Social Work and/or Masters of Counseling required
- Must have current CPR.
