
Rn Case Manager

- Collaborate with interdisciplinary teams to assess, plan, implement, and evaluate patient care needs.
- Coordinate discharge planning and transitions of care, ensuring safe and timely movement to post-acute settings.
- Serve as a liaison between patients, families, providers, and external agencies to support continuity of care.
- Identify barriers to discharge and work proactively to resolve them.
- Maintain accurate documentation in accordance with regulatory and organizational standards.
- Participate in daily multidisciplinary rounds and unit huddles to align care plans.
- Advocate for patient needs while balancing clinical, financial, and operational considerations.
- Current RN license in WA State or Compact License (CCM or ACM)
- Basic Life Support (BLS) certification
- Minimum of 1 year of acute care nursing experience
- Strong knowledge of discharge planning, utilization review, and care coordination principles
- Excellent communication, critical thinking, and organizational skills
- Familiarity with EMR systems and documentation standards
- Experience in case management within Acute Hospital and/or Long Term Acute Care/Rehab/Skilled Nursing settings
- Proficiency with EPIC, utilization review, care coordination, and discharge planning
- Experience in ED, Labor & Delivery, NICU, and working with children is preferred
- 4x10s and 5x8s:
- Monday through Friday, between 7:00 AM and 5:30 PM
- 3x12s:
- Day Shift:7:00 AM – 7:30 PM
- Night Shift:7:30 PM – 7:00 AM
