As a fiscal sponsor of the Morgan County Interagency Oversight Group (MCIOG), Trailhead provides administrative oversight for MCIOG staff, who will work most closely with members of the [MCIOG and the Board. To learn more, check out .About The Morgan County Interagency Oversight Group (MCIOG)The Morgan County Interagency Oversight Group, (MCIOG), is a Collaborative Management Program funded by the state of Colorado. It was established in Morgan County in 2008 to support youth and families who are working with multiple agencies or at risk for working with multiple agencies and who are at risk for truancy, drug and alcohol abuse, behavior issues, mental health issues and more.Position SummaryThe Part-Time (24 hours/week) Family Support Partner (FSP) position is designed to provide intensive levels of direct support for families. The Family Support Partner will report to the Coordinator & High-Fidelity Wrap Around (HFW) Supervisor and will work closely to support positive outcomes for the families. Often, but not always, the FSP is a graduate of HFW and should be a family member of a person with complex emotional or mental health needs and system experience. Family support partners add value to HFW for several reasons. First, as someone who has experienced similar challenges, they can engage families at a deeper level than people without this experience. Second, they provide direct support to implement the theory of change. The FSP may be able to work deeply to help families identify and engage some natural supports in the process as it moves forward. FSPs also help families navigate uncomfortable environments.This position will be classified as an Hourly Employee at Trailhead Institute. This position will work an in-person schedule with regular in-office hours.Responsibilities:
  • Orient and help the families understand how HFW can help address their needs, including explaining how the HFW approach is different from other system involvement methods
  • Ensure the families understand consent and release forms
  • Gather information for the Strengths, Needs and Culture Discovery and helps families prepare for their first team meeting
  • Helps the team understand the importance of family voice and choice, family culture and strengths, and advocates for options that align
  • Ensures the families understand the functional assessment and crisis prevention planning process
  • Regularly check in with the families to ensure their voice is being heard, their plans are realistic and if necessary, support in completing the families' action steps
  • Maintain continued contact with the families regarding plans and action steps, and if the plan is not working, communicate with the Supervisor about next steps
  • Encourage families to complete steps using the "do for, do with, cheer on" approach and supports youth and families to take on increasingly active roles in HFW meetings and build self-efficacy
  • Support families in identifying needs that will continue to require intervention after formal HFW ends and noting lessons learned through HFW
  • Identify a culturally appropriate way to celebrate families' graduation from HFW
  • Obtain releases of information and ensure confidentiality of individual information
  • Model personal responsibility, self-advocacy, creative thinking and hope
  • Provides culturally competent care that aligns with families' culture and strengths
  • Exhibits a non-judgmental approach, effective listening, and positive interactions
  • Complete all documentation required in a timely and legible manner
  • Collect and present data collection and management that may be required to the IOG Coordinator in a timely and legible manner
  • Complete all required system of care training
  • Other duties as assigned
Desired Experience:
  • Hold a high school diploma or GED
  • Broad base of life experience and possess a diverse view of what families need to live better lives
  • Lived experience caring for a child or youth with serious mental health challenges
  • Understanding of and experience with different systems, including schools, mental health, child welfare, juvenile justice, health, etc.
  • Knowledge of community resources and an ability to engage with resources directly
Skills
  • Ability to collaborate closely with teams of family members, natural supports and professionals to develop and carry out individualized plans
  • Must be able to work and engage with people from diverse cultures, ages and backgrounds
  • Must be able to establish healthy boundaries
  • Excellent communication skills, both written and verbal
  • Ability to troubleshoot, take initiative, be forward thinking to support families
  • Familiarity in Microsoft Office and Zoom, as well as the aptitude to learn new computer applications as needed
  • Ability to adapt communication style to meet the needs of a variety of internal and external stakeholders
  • Keen analytic, organization and problem solving/decision-making skills
  • Demonstrated work history of coordinating and completing tasks independently and on time
  • Excellent interpersonal and conflict resolution skills, able to resolve issues directly as they arise through the practice of ethical communication
  • Bilingual Spanish preferred

Trailhead Institute offers a generous benefits package including paid vacation and sick time (prorated for part-time employees), health, dental, vision, life, short-term and long-term disability, a free RTD EcoPass, and a 401k with a 100% employer match up to 10% of compensation.The salary for this position is provided by limited-term funding. If the funding ends, the position may be discontinued, and there is no guarantee for future employment with Trailhead Institute.License and Proof of Insurance RequirementIf you will be using a personal vehicle to drive yourself for Trailhead Institute business, you must provide a valid driver license and proof of insurance.
