Responsibilities
  • Assures spiritual assessment of Seniors/clients and families/caregivers in the hospice program and appropriate services and rendered in a timely manner.
  • Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual and bereavement needs.
  • Provides direct spiritual care to Seniors/clients and families/caregivers.
  • Provides for funeral or memorial services for Seniors/clients as requested.
  • Plans, coordinates, and implements the volunteer training program and the volunteer orientation process in an effective and efficient manner.
  • Oversees timely collection, storage, and retrieval of volunteer data including monthly, quarterly, and annual reports, progress notes, evaluations, and criminal background checks.
  • Assures volunteer hours are maintained at a level that satisfies State, Federal, and Tabitha Hospice policies.
  • Provides support and guidance to volunteers on a regular basis and when difficulties arise.
  • Plans, implements and maintains the Bereavement program in order to meet the needs of those served.
  • Coordinates services to include the provision of the assessment and delivery of grief counseling needs and services (one-to-one, groups, mailing program and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers for a minimum of 13 months following client's death.
  • Responsible for the delivery of bereavement services, as assigned..
  • Provides crisis intervention for Hospice or community bereaved.
QualificationsCOMMUNICATION: Must be able to speak, read, write, and understand the English language as well as terminology and jargon specific to the job.PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job. Must have unrestricted lift/push/pull ability of 25 - pounds, demonstrating strength and agility at Tabitha’s discretion.AGE REQUIREMENT: At least age 18.EDUCATION: Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.EXPERIENCE: Minimum of three years combined experience in marketing, program coordination, or staff scheduling and supervision.CREDENTIALS: Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.EEO Disability/Veteran
