- Assures spiritual assessment of Seniors/clients and families/caregivers in the hospice program and appropriate services and rendered in a timely manner.
- Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual and bereavement needs.
- Provides direct spiritual care to Seniors/clients and families/caregivers.
- Provides for funeral or memorial services for Seniors/clients as requested.
- Plans, coordinates, and implements the volunteer training program and the volunteer orientation process in an effective and efficient manner.
- Oversees timely collection, storage, and retrieval of volunteer data including monthly, quarterly, and annual reports, progress notes, evaluations, and criminal background checks.
- Assures volunteer hours are maintained at a level that satisfies State, Federal, and Tabitha Hospice policies.
- Provides support and guidance to volunteers on a regular basis and when difficulties arise.
- Plans, implements and maintains the Bereavement program in order to meet the needs of those served.
- Coordinates services to include the provision of the assessment and delivery of grief counseling needs and services (one-to-one, groups, mailing program and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers for a minimum of 13 months following client's death.
