Village Health Partners is a well-established primary care practice dedicated to providing compassionate and comprehensive healthcare services to our community. We have offices in Plano, Frisco, and McKinney.We are currently seeking an experienced Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team of providers. You will assess, diagnose, and treat a wide range of acute and chronic health conditions, provide preventive care and health education, and collaborate with physicians to coordinate patient care. Qualified candidates must have recent FNP or PA experience in a primary care or family medicine setting.Responsibilities:
  • Conduct comprehensive health assessments, including medical history review, physical examinations, and diagnostic tests
  • Diagnose and treat acute and chronic illnesses, injuries, and conditions across the lifespan
  • Develop and implement individualized treatment plans
  • Prescribe medications, order and interpret diagnostic tests, and perform minor procedures
  • Provide preventive care services, including health screenings, immunizations, and health promotion counseling
  • Educate patients and families about health maintenance, disease prevention, and self-care management strategies
  • Collaborate with physicians to coordinate patient care and ensure continuity of care
  • Document patient encounters accurately and timely in EMR
  • Adherence to HIPAA rules and regulations
Qualifications:
  • Current Texas license and certification
  • Graduate of an accredited FNP or PA program
  • 2 or more years experience as a Family Nurse Practitioner or Physician Assistant in primary care or family medicine preferred
  • Live within a 45 minute commute to Plano, Frisco, McKinney
  • Excellent clinical assessment, diagnostic, and treatment skills
  • Proficiency in using electronic medical record (EMR) systems and medical software applications
  • Ability to work independently and as part of a collaborative team in a fast-paced clinical environment
  • Experience in properly handling confidential and sensitive information; knowledge of HIPAA rules and regulations
